By signing this document, I acknowledge that:

• I have had an opportunity to read the Plamedll Terms and Conditions.
• I understand that the $50 booking payment is credited toward my agreed commission price.
• I understand that the remaining balance is payable after completion and before shipment.
• I understand that the scheduled consultation day is also the intended artwork start date.
• I understand that artwork production takes approximately two weeks, subject to the stated conditions.
• I understand that shipping is estimated at 5–10 business days after dispatch and standard shipping is included in the agreed commission price.
• I understand the copyright and personal-use provisions.
• I understand the cancellation, refund, revision, and delivery policies.
• I understand that permission to publish my identifiable commissioned artwork is a separate, voluntary choice.

4. Publication Consent Record

My selection in the official Plamedll booking form is recorded as:

This section records the customer's selection. It does not replace the separate Yes/No consent question in the booking form. The customer's actual recorded selection must be retained with the commission records.

5. Customer Signature

My signature confirms that I have reviewed the commission details and acknowledge the applicable Plamedll policies. I understand that this document does not remove any rights that cannot legally be waived.

6. Artist Confirmation

Plamedll confirms receipt of the commission request and will proceed according to the agreed details, subject to payment and the applicable policies.
